Potter County Human Services is seeking a highly motivated individual to become part of our team as a Case Management Specialist.
The Case Management Specialist will conduct client evaluations utilizing American Society Addiction Medicine criteria and determine a level of care, orients clients to the program on policy, procedures, and treatment process. Provides case coordinations for Medically Assisted Treatment clients, completes quarterly and monthly reports. Also, works with other agencies to coordinate services and attends meetings and trainings.
