If you are a practitioner of medicine or osteopathy, or a physician assistant licensed by the Board of Medicine, or an advanced practice registered nurse jointly licensed by the Board of Medicine and the Board of Nursing, you are eligible to issue a written certification recommending the use of medical cannabis to a qualifying patient.
To create an account in the medical cannabis portal, click the link below. Upon creating an account, you will receive access to the written certification form. If you need help with enrollment, click “help with enrollment” below.
Practitioners must log into the medical cannabis portal to complete a digital written certification for the patient. The patient will then have access to a digital card validating their written certification for the use of medical cannabis.
A written certification for a minor or vulnerable adult should include parent or legal guardian information so they are able to pick up medical cannabis on the patient's behalf. If the patient or parent/legal guardian wishes to have a registered agent, be sure to include their information. Up to two parents/legal guardians and one registered agent are allowed on each certification.

If you elect to issue manual written certifications rather than utilizing the medical cannabis portal, please email MedicalCannabis@cca.virginia.gov to request the form. Include your full legal name and license number with your request.
